Donna (Diane) Formaro Obituary

Donna (Diane) Formaro, 84, of West Des Moines, passed away on Thursday, January 7, 2021, at Fountain West Health Care Center in West Des Moines.

Donna was born on November 23, 1936 in Des Moines, IA, to Junior and Marian (Wright) Clemens. She lived most of her life on the eastside of Des Moines and was a graduate of East High School. Donna worked as a waitress at Woolworth's downtown where she met her husband Phillip and married in September of 1964.

Donna was a homemaker for most of her life and a member of St. Joseph’s Catholic Church.

Donna enjoyed going dancing with her husband Phillip, doing word searches, family dinners, holidays and going to the Iowa State Fair. She also enjoyed visiting her sister and family in Georgia. She loved God, her family, and especially spending as much time as she could with her best friend and granddaughter Makayla.

She was preceded in death by her parents; husband Phillip, sister Marlene and brother Bill.

Donna is survived by her daughters, Michelle (Mike) Chambers, Teresa (Shawn) Terry, grandchildren; Makayla, Dawn, Schauna and Michael, six great grandchildren, sister Marilyn Hall-Cook (Bruce) and several nieces and nephews.

A visitation will be held from 2-5 p.m. Saturday, January 16, 2021 at the Iles Grandview Park Chapel. Burial will be at a later date at Glendale Cemetery in Des Moines.
